A bold approach to optimizing wellness and equity and addressing social influencers of health. Did you know that “health care” accounts for only 20% of a person’s health and well-being?

Trinity Health includes a focus on community health and well-being (CHWB) as a key pillar of its integrated people-centered health system. Including much more than traditional community benefit; CHWB calls for Trinity Health to improve the health of the entire population living in the communities it serves.

In the United States, health care spend is almost entirely focused on clinical care, not preventative or public health. However, clinical care and genetics account for only 30 percent of the factors that impact the health of populations. To deliver the Triple Aim for populations and communities, Trinity Health as a people-centered health system addresses all influencers of health, not just episodic clinical health care.

Trinity Health is one of the largest not-for-profit, faith-based health care systems in the nation.
